My 30-month-old toddler (weighing 10.7 kg) has had a fever staying above 38.5°C (101.3°F) since this morning. We saw a doctor earlier who prescribed cold medication and advised a 4 mL dose of fever reducer. Right now, their temperature has spiked to 40.2°C–40.3°C (around 104.5°F), but they are still running around, playing, and acting completely fine. I have always heard that a fever over 40°C requires a trip to the emergency room. Should we take them to the ER right away?

Answer:

It is completely understandable to feel alarmed when you see a number like 40.2°C on the thermometer. However, the most reassuring sign right now is how well your child is acting.


Assessing the Situation

  • Overall condition matters more than the number: Even with a fever above 40°C, a child who is energetic, alert, drinking fluids, and urinating normally is generally coping well with the infection.

  • Emergency criteria depend on warning signs, not just a number: While a high fever warrants close observation, an emergency room visit is determined by the presence of red flag symptoms rather than simply crossing the 40°C mark.


What to Do at Home Right Now

  • Administer the fever reducer: Give the recommended 4 mL dose and recheck their temperature in 30 to 60 minutes to see if it starts trending downward.

  • Keep them comfortable: Dress your child in a single layer of lightweight clothing and maintain a comfortably cool room temperature.

  • Prioritize hydration: Offer small sips of water or an oral electrolyte solution frequently (a sip or two every 5 to 10 minutes).

  • Avoid sponge baths: Do not use cold or lukewarm water to wipe them down, as this can trigger shivering, which raises internal body temperature and causes discomfort.


When to Seek Immediate Emergency Care

Go to the nearest emergency department or call emergency services if your child develops any of the following red flags:

  • Difficulty breathing (breathing much faster than usual, grunting, or the chest and ribs sucking in deeply)

  • Pale, grey, or bluish lips, tongue, or nail beds

  • Extreme lethargy, difficulty waking up, confusion, or inability to make eye contact

  • Signs of dehydration, such as no wet diapers or urination for over 8 hours

  • A stiff neck, signs of severe headache, or repeated vomiting

  • A rash of tiny purple or red spots that does not fade when pressed firmly


When to Schedule a Pediatric Follow-Up (Non-Emergency)

Contact your pediatrician for an in-person evaluation if:

  • The fever lasts for more than 3 consecutive days.

  • Their fluid intake or energy levels drop to less than half of their normal baseline.

  • They become unusually irritable, inconsolable, or begin complaining of localized pain (such as an earache or throat pain).


As long as your child remains playful, well-hydrated, and free of emergency warning signs, it is safe to manage the fever with medication and monitor them at home. If their condition declines or you feel uncertain, consult your pediatrician or seek immediate medical care.
