I accidentally gave my 22kg child a double dose of fever reducers. I administered a combination of Ibuprofen (10mg) and Acetaminophen powder at 4 PM and again at 9 PM (a 5-hour interval). My child's temperature has now dropped significantly from 38.7°C to 36.0°C. My child is currently sleeping peacefully, and it doesn't seem like severe hypothermia.


However, since they are still young, I am worried that this might have put a heavy burden on their liver or kidneys. I will continue to monitor their body temperature overnight, but what kind of side effects should I look out for? Should I take my child to the clinic for a blood test first thing in the morning?

I completely understand your concern regarding the accidental double dosing of your child's fever-reducing medications.


Current Situation Evaluation

It is a relief that your child's temperature has stabilized from 38.7°C down to 36.0°C. A body temperature of 36.0°C is well within the normal range and is not considered severe hypothermia. Please be assured that a single event of duplicate dosing is highly unlikely to cause serious or permanent damage to a child's liver or kidneys.


Immediate Things to Check

  • Is your child sleeping comfortably with regular breathing?

  • Is their skin color normal and do they feel warm to the touch?

  • If you gently wake them, is their reaction typical?


Overnight Monitoring Guidelines

  1. Temperature checks: Monitor their temperature every 2 to 3 hours. Pay extra attention if it drops below 35.5°C.

  2. Hydration: Offer small sips of water whenever your child wakes up.

  3. Temperature regulation: Adjust their blankets as needed to help them maintain a comfortable body temperature.


When to Visit a Doctor

Please seek immediate medical evaluation if you notice any of the following red flag symptoms:

  • Vomiting or severe abdominal pain

  • Lethargy, confusion, or extreme difficulty waking up

  • Body temperature remaining below 35.0°C continuously

  • A significant decrease in urine output


Morning Action Plan

  • If your child wakes up acting completely normal without any of the warning symptoms mentioned above, a blood test is unnecessary.

  • Encourage them to drink plenty of fluids throughout the day to help flush the medication out of their system.

  • Withhold any further fever reducers. If a fever returns and medication is absolutely necessary, wait at least 8 hours from their last dose.


If your child is currently resting peacefully, please try not to worry too much. Continue your overnight monitoring and consult your pediatrician if any abnormal symptoms develop or if you remain concerned about their condition.
