My 5-year-old child has had a persistent low-grade fever. On Saturday morning, their forehead felt warm at around 37.3°C, but it quickly returned to normal. They seemed fine on Sunday. However, since Monday, the temperature pattern has been: 36.9°C in the morning, around 37.3°C in the afternoon, and 37.7°C in the evening. Once they fall asleep, their temperature drops back to normal until morning.


There is no runny nose or cough. Could their throat be swollen? Usually, a swollen throat causes a high fever over 38°C at night, but that isn't happening this time. My child is in great condition, playing and eating very well. Today, the same temperature pattern repeated (it was 37.7°C around 8 PM, but dropped to 37.0°C after falling asleep). Should I take my child to the clinic tomorrow?


Answer:

It is completely understandable to be worried about a recurring low-grade fever, especially when you notice a consistent daily pattern.


Based on the temperature fluctuations you described:

• Morning: Normal (36.9°C)

• Afternoon: Mildly elevated (37.3°C)

• Evening: Peaking at 37.7°C

• During sleep: Returns to normal


This daily temperature variation can actually be a normal physiological phenomenon. Even perfectly healthy children naturally experience a temperature rise of 0.5 to 1 degree Celsius during the late afternoon and evening.


Current Assessment

There are many positive signs in your child's current situation:

• No high fever (over 38°C)

• Excellent general condition (playing and eating well)

• No respiratory symptoms

• Temperature normalizes during sleep


Recommendations

  1. Observe for now: Since your child is active and symptom-free, this is not an urgent medical situation. You can safely monitor them for another day.

  2. Focus on condition over temperature: Pay more attention to your child's overall behavior and mood rather than the numbers on the thermometer.

  3. Ensure hydration: Keep your child well-hydrated by offering plenty of water and water-rich fruits.


When to See a Doctor

Please seek an in-person medical evaluation if your child develops any of the following:

• Fever rising above 38.5°C

• Lethargy, lack of energy, or a sudden decrease in appetite

• Complaints of a sore throat or difficulty swallowing

• New symptoms such as a rash or stomachache

• The low-grade fever persists for more than 5 days


Currently, the situation does not appear to be serious. However, for your peace of mind and to rule out any mild throat inflammation, it is perfectly fine to visit your pediatrician tomorrow for a brief check-up and physical examination.
