Question:

My 8-month-old baby had diarrhea about five times, so we visited the clinic, and the doctor diagnosed it as acute gastroenteritis. That afternoon, my baby developed a fever of 38°C (100.4°F), so I gave acetaminophen. The fever dropped after two hours but returned five hours later, which has been repeating for two days. We went to the clinic again, and the doctor noted that my baby's throat was a bit swollen, so I was instructed to give cold medicine along with the fever reducer.

On the morning of the third day, the fever rose again. I gave the fever reducer, but the temperature wouldn't drop below 37.5°C (99.5°F), and a mild fever persists. If the fever spikes again, is it okay to give a different class of fever reducer? Also, my baby naturally hates drinking water and completely refuses it by batting the cup away. Is it acceptable to use formula for hydration instead of water?

Answer:

It must be very worrying for you that your 8-month-old baby is dealing with both gastroenteritis and a cold at the same time, especially with the fever repeating for three days.

【Alternating Fever Reducers】

Yes, you can use a different class of fever reducer (Ibuprofen).

• This is recommended when the effect of acetaminophen is insufficient.

• Ibuprofen is safe to use for babies 6 months of age and older.

• You can alternate the two different medications at 3-4 hour intervals.

• Please make sure to strictly adhere to the specific dosing intervals for each medication (Acetaminophen every 4-6 hours, Ibuprofen every 6-8 hours).

【How to Hydrate】

Yes, you can absolutely hydrate your baby with formula!

• During a bout of gastroenteritis, mix the formula so it is more diluted than usual (add a little extra water).

• Offer small amounts frequently (30-50ml every 30 minutes).

• If the diarrhea is severe, an oral rehydration solution (electrolyte solution) is highly recommended.

• Try giving fluids little by little using a spoon or a medicine dropper.

【Warning Signs to Watch For】

→ Go to the Emergency Room immediately if: Your baby hasn't urinated for 8 hours or more, cries without producing tears, or is severely lethargic/unresponsive.

→ Visit your doctor again if: A high fever lasts for 4 days or more, diarrhea worsens, or there are obvious signs of dehydration.

Having a cold and gastroenteritis simultaneously can make the recovery process a bit slower. Adequate hydration and plenty of rest are the most important things right now. I hope your baby feels better very soon.
