Q: My 15-month-old baby has had a high fever for 8 days without any other symptoms. What could be the cause?

Question:

Hello, I am asking about my 15-month-old baby who has had a high fever (38-40°C) for 8 days without any other symptoms.


I initially alternated over-the-counter fever reducers, and on the 3rd day, I visited a clinic to get prescribed fever medicine. The baby started having diarrhea that day. On the 6th day, a rash or hives appeared all over the torso. The fever intervals started to widen, and the peak temperature dropped, so I thought it was ending. However, the fever continued every 6-7 hours, so I visited the clinic again the next day.


The doctor said all test results were normal and the cause was unknown, but added antibiotics to the prescription. Flu and COVID tests were negative, and blood and urine test results were normal. Lung sounds, ears, and lymph nodes are all fine. What could be the reason?


The prescribed medicine is meant to be taken 3 times a day. My sleeping baby currently has a fever again (38.2°C). Since the baby is sleeping well, I haven't given any medicine, but the fever isn't dropping. I am worried and wondering if I should arbitrarily give a different type of fever reducer I have at home right now.

You must be very worried that your 15-month-old baby has had a high fever for 8 days. It must be even more anxiety-inducing since the fever continues despite all test results being normal.


Current Situation Assessment:

• High fever of 38-40°C lasting for 8 days

• Fever pattern improved after a whole-body rash on the 6th day

• All test results are normal

• Prescribed additional antibiotics


This pattern is highly likely to be a viral illness (such as roseola, enterovirus, etc.). The appearance of a rash after the fever is particularly characteristic.


Right now:

1. If the baby is sleeping well at 38.2°C, do not wake them up to give medicine.

2. Give the prescribed fever reducer if the temperature rises above 39°C or if the baby is uncomfortable.

3. Check the last dosage time → You can give it if 4-6 hours have passed.

4. Hydration is the most important - frequently offer water/ion drinks when the baby is awake.


⚠️ Go to the emergency room immediately if:

• Energy levels drop rapidly

• No urination for more than 8 hours

• The rash turns purplish and does not fade when pressed

• The neck is stiff or the baby is sensitive to light


Most viral high fevers improve within 7-10 days. If the fever persists tomorrow morning, get re-examined, and discuss a repeat blood test or a referral to a secondary hospital.
