Question:

My 5-year-old child (16 kg) has been taking Ramnos powder for skin health and constipation, but we completely ran out. We only have Ramnos 250 mg capsules at home. Can I open the capsule and give just the powder to my child, or is that not recommended?

Answer:

It is best to avoid opening the capsule to feed your child the powder inside. While it may seem like a simple alternative, capsules and powders are formulated differently, and altering the capsule can affect how well the medication works.

Difference in Formulation and Delivery

  • Powder form: Formulated to dissolve easily and remain stable upon oral administration.

  • Capsule form (250 mg): Enclosed in a specific capsule shell designed to protect the active ingredients from stomach acid so they can be delivered directly into the intestines where they are most effective.

What Happens if You Open the Capsule?

  • Decreased Effectiveness: Once the protective capsule shell is removed, the contents are exposed to stomach acid immediately. This can diminish the viability of the active probiotics before they reach the intestines, making the treatment much less effective.

  • Taste Issues: The raw contents inside a capsule are often bitter or unpleasant in flavor, which may cause your child to reject future doses or develop an aversion to taking medication.

What You Should Do:

  • Obtain a refill of the powder: The best course of action is to contact your pediatrician or local pharmacy to get a new supply of the powder formulation that was originally prescribed.

  • Pausing for a day or two is safe: Supportive supplements for skin and mild constipation are not acute life-sustaining medications. Skipping a dose for a day or two until you can pick up the correct powder formulation is completely safe and will not cause immediate harm.

  • Swallowing the capsule whole: If maintaining daily intake is necessary and your child is comfortable swallowing pills, a 5-year-old can sometimes swallow small capsules with plenty of water or a spoonful of yogurt or applesauce. However, never force a young child to swallow a capsule if they are not accustomed to it, as this poses a choking hazard.

Opening the capsule is not recommended. If you have questions about adjusting your child's treatment or finding an immediate substitute, consult your pediatrician or pharmacist for an appropriate pediatric alternative.
