Question:

We are currently traveling abroad, and my older child (29.5 kg / approx. 65 lbs) has developed a worsening cold with thick, yellow nasal discharge. We have a 2-hour flight back home tonight and cannot visit a clinic until tomorrow. I have an extra powdered antibiotic that was prescribed for my 5-year-old (16.4 kg / approx. 36 lbs). Since my older child weighs almost double, is it safe to give double the dose by mixing two packets into the liquid? Also, I am worried about severe ear pain during the flight due to heavy nasal congestion. Would giving a pain or fever reducer before boarding help?

Answer:

Traveling with a sick child can be very stressful, but safety must come first regarding medications.

Never adjust or share antibiotic dosages on your own

  • It is not safe to calculate and give a medication prescribed for a younger sibling to an older child based solely on weight.

  • Antibiotics require precise dosing based on the specific type of infection, the exact medication concentration, and individual clinical assessment. Giving an arbitrary dose risks ineffective treatment, promoting antibiotic resistance, or causing harmful side effects.

  • The safest course of action is to withhold the antibiotic and take your child to see a doctor immediately after you return home tomorrow.

Managing nasal congestion and ear pressure before and during the flight

A severe cold can cause the Eustachian tubes in the ears to swell, making it difficult to equalize middle ear pressure during takeoff and landing. Here is how you can help keep your child comfortable:

  • Saline Nasal Spray: Use a sterile saline nasal spray or drops frequently before the flight. This helps thin thick mucus and reduces swelling around the Eustachian tube openings.

  • Pain/Fever Reliever: Giving an age- and weight-appropriate dose of an over-the-counter pain reliever (such as acetaminophen or ibuprofen) about 30 to 45 minutes before boarding can help reduce ear pain and general discomfort during the flight.

  • Active Swallowing During Pressure Changes: Encourage your child to sip water, chew gum, or swallow frequently during takeoff and especially during descent, when ear pressure changes are most dramatic.

  • Hydration: Offer plenty of fluids to keep the respiratory mucous membranes hydrated.

Signs that require immediate medical attention

Having yellow nasal discharge alone typically does not prevent a child from flying. However, consult airport medical services or delay travel if you notice:

  • A persistent high fever above 38.5°C (101.3°F)

  • Difficulty breathing, wheezing, or unusually rapid breathing

  • Severe, intolerable ear pain

  • Extreme lethargy or poor responsiveness

Next Steps:

Focus on supportive care for today—use saline spray, maintain hydration, and administer weight-appropriate pain relievers before the flight to ease ear discomfort. Do not use the leftover antibiotics. Schedule an in-person visit with your pediatrician as soon as you arrive home so your child can receive an accurate diagnosis and appropriate prescription.
