Question:

Yesterday after daycare, my child had a fever of 38.8°C. We alternated fever reducers, and the temperature dropped to 37.5°C, but it spiked again at dawn, so we visited the clinic. My child had some nasal congestion while sleeping and complained of an upset stomach, but there was no vomiting or diarrhea at that time. I forgot to mention the upset stomach during the consultation. The doctor diagnosed sinusitis and a swollen throat, and prescribed antibiotics and other medications.

However, after coming home, my child suddenly vomited and had loose stools. I also heard that classmates at the daycare have the stomach flu (enteritis). Can a doctor detect the stomach flu just by listening with a stethoscope? I have an appointment for tomorrow, but should I stop giving the medication prescribed today? Do I need to get new medication for the stomach flu?

Answer:

It is completely understandable that you are worried, especially seeing your child suddenly experience vomiting and loose stools while knowing there is a stomach flu outbreak at their daycare.

Stethoscope and Stomach Flu Diagnosis

It is very difficult to definitively diagnose the stomach flu (enteritis) using only a stethoscope. While listening to the abdomen might reveal increased or decreased bowel sounds, this alone is not enough for a confirmed diagnosis. The stomach flu is primarily diagnosed based on clinical symptoms (such as vomiting, diarrhea, and abdominal pain) and the patient's recent medical history.

Evaluating the Current Situation

Given that your child developed vomiting and loose stools after starting antibiotics for sinusitis and an inflamed throat, there are two main possibilities:

  1. This is a gastrointestinal side effect from the newly prescribed antibiotics.

  2. This is a new stomach flu infection, especially considering the recent exposure at daycare.

Immediate Steps to Take

  1. Prioritize Hydration: Offer water or oral rehydration solutions frequently in very small sips to prevent dehydration.

  2. Pause Medication: It is advisable to temporarily stop the medication prescribed today until you can consult your doctor again.

  3. Dietary Adjustments: Offer bland, easy-to-digest foods like the BRAT diet (bananas, rice porridge, applesauce, toast) if your child is hungry.

  4. Monitor Symptoms: Keep a record of the frequency and amount of vomiting and diarrhea.

Warning Signs (Seek Emergency Care)

Please take your child to the emergency room immediately if you notice any of the following:

• No urination for more than 8 hours.

• Inability to keep any fluids down due to continuous, repeated vomiting.

• Bloody stools or severe abdominal pain.

• Severe lethargy, unresponsiveness, or decreased consciousness.

Please keep your scheduled clinic appointment for tomorrow. Be sure to explain the sudden onset of vomiting and diarrhea to your doctor so they can determine whether the antibiotics need to be changed or if a new treatment plan for the stomach flu is required. If symptoms worsen significantly before your appointment, do not hesitate to seek emergency medical care.
