Question:
My second-grade daughter has had a fever for over two full days. On the first and early second days, her temperature would rise quickly even after taking a fever reducer. Today, however, the interval has lengthened, and her temperature goes back up to about 38°C after 5 to 6 hours. Her highest temperature was 39°C. She has taken three flu tests, and all came back negative. She has absolutely no cold symptoms other than the fever. The pediatrician thinks it might be a simple viral fever, but if she continues to have a fever tomorrow, what additional tests should we do?
Answer:
You must be very worried as your second-grade daughter's fever has lasted for 3 days. It is especially anxiety-inducing when there is only a fever without any other cold symptoms.
Current Status Evaluation:
• The fever pattern is improving (the duration of the fever reducer's effect has increased).
• Three flu tests were negative.
• No other respiratory symptoms are present.
Considerations for additional tests if the fever persists tomorrow:
1. Blood tests (CBC, CRP) - To check for bacterial infection.
2. Urine test - To rule out a urinary tract infection (especially important for girls).
3. Chest X-ray - To check for early-stage pneumonia.
4. COVID-19 test - If you haven't done one yet.
Go to the Emergency Room immediately if you notice:
→ Difficulty breathing, decreased consciousness, or a severe headache.
→ New symptoms like abdominal pain or a rash appear.
→ She has not urinated for over 8 hours.
Home Care:
• Ensure adequate hydration (about one cup per hour).
• Cross-dosing of fever reducers is possible (Acetaminophen/Tylenol and Ibuprofen).
• Dress her in light clothing.
