Question:

Is this hives? I have a 24-month-old boy. He was perfectly fine this morning, but after coming home from daycare today, I noticed what looks like a rash or hives only on his stomach. Looking at his daily menu, there was nothing that could have triggered an allergic reaction. He doesn't seem itchy, as he isn't scratching it at all. Could this be a heat rash, or is it hives? Clinics are already closed for today, so would it be okay to wait and see a doctor tomorrow?

Answer:

It must be worrying to see a sudden rash on your 24-month-old child. It is a relief that it has only appeared on the stomach and does not seem to be itchy.

【Current Condition Assessment】

A rash confined to the stomach without itchiness is not an emergency. Hives are usually itchy and appear on various parts of the body. If your child is not scratching, it is highly likely to be a heat rash or a mild skin reaction.

【Things to Check Immediately】

• Shape of the rash: Small water-like bumps (heat rash) vs. swollen, raised lumps (hives)

• Is it spreading to other areas of the body?

• Is there any fever or difficulty breathing?

• Is there any swelling of the lips or tongue?

【Management for Tonight】

1. Maintain a cool environment (set the air conditioner to 24-26°C).

2. Change his clothes into breathable cotton wear.

3. Give him a light shower with lukewarm water.

4. Take clear pictures of the rash (show these to the doctor during tomorrow's visit).

⚠️ Go to the Emergency Room IMMEDIATELY if you observe:

→ Swelling of the face/neck, difficulty breathing, or the rash spreading rapidly across the whole body.

→ Vomiting, dizziness, or a decrease in consciousness.

If there is no itchiness and the rash remains only on his stomach, it is perfectly fine to wait and see a doctor tomorrow. Please carefully observe him for any changes overnight!
