I am a mother of a 25-month-old toddler. He suddenly started running a fever at 7:00 PM, which quickly reached 39.5°C. I immediately gave him 4.5ml of ibuprofen. By 8:30 PM, his temperature was 38.7°C, and by 9:04 PM (about two hours after taking the medication), it had dropped to 38.3°C. Since two hours have passed, should I cross-dose and give him a different fever reducer now?

Answer:

It is completely understandable to be worried when your little one has a sudden high fever. The fact that his temperature is dropping from 39.5°C to 38.3°C after taking ibuprofen is a very good sign.

Current Situation Assessment:

• The ibuprofen is working well, as evidenced by a 1.2°C drop in about two hours.

• A temperature of 38.3°C is entering a much safer, milder fever range.

• The most important factor right now is whether your child looks and acts comfortable.

Should You Cross-Dose Now?

You do not need to cross-dose (alternate with another medication) right now.

• His fever is currently on a downward trend.

• The effects of ibuprofen typically last for 6 to 8 hours.

• If your child is playing well and able to drink fluids, you should simply observe him.

When Cross-Dosing is Necessary:

• If his fever spikes again to 39°C or higher and he is visibly struggling.

• If it has been at least 4 hours since the ibuprofen dose and he remains very uncomfortable.

• In these specific cases, you can administer acetaminophen (such as Tylenol) at the appropriate weight-based dosage.

Important Precautions to Keep in Mind:

• Maintain a minimum 6-hour interval between doses of ibuprofen.
